What information should I get from the father of my baby before he leaves?

I'm 8 weeks pregnant and the father of the baby has decided not to be involved. It's a long story.

We're meeting tomorrow to talk about some details but I don't think I'll see him after that.

Is there anything that he should sign? What kind of information should I ask him for?

I don't plan on filing for child support or anything like that, but I know I should have information about him. What information should I get from the father of my baby before he leaves?Medical History.

Thats all you need I guess if you don't plan on suing for Child Support. What information should I get from the father of my baby before he leaves?
Medical history of him and his family. That's really all you need if you don't want child support. What information should I get from the father of my baby before he leaves?Let the court deal with it and then you won't have to worry about all that.

-EDIT-

Completely agree with Baby Gabriella. You should go through the court to protect you and your child just in case. What information should I get from the father of my baby before he leaves?
My question: Why don`t you want child support???? It`s his child too, and he has to be responsible financially too. You may be all right now financially, but who knows what will happen in the future. You have to protect yourself legally!

Edit: It is so easy for a guy to say, that he decided not to be involved! What information should I get from the father of my baby before he leaves?Family medical history is very important. Also, address and all that in case of a medical emergency in which he could be of service (blood transfusion, organ donating etc.) Blood type would be good.

There is nothing he can sign at this point. All that has to wait until after baby is born. What information should I get from the father of my baby before he leaves?
Get contact information in case of an emergency. Also get his health history and his family history. It will be important for the child to know this information in the future.

If you're not planning on filing child support or anything and you are completely happy without him being involved at all, I can't really think of anything else you could need.

There's really nothing he can sign unless you get a notorized paper saying that he understands that he is the father and you understand that you are taking full custody and full responsibility for your pregnancy and your child's care. This would probably need to be drafted by a lawyer.

One thing that could come back on you, however, is if he decides in a few years, or even in 15 years to be involved and demand rights to his children. Unless you and he see a lawyer and draft something wherein he completely withdraws all legal rights to the child, this will be a consideration until the child is 18.What information should I get from the father of my baby before he leaves?
Medical History- for family. I would suggest as the others have that you should just go Thur the courts with that. Just a reminder, I am not sure where you live. In some states, if a mother wants to file for help ie - food stamps, money, health insurance etc they need the fathers name and location Information. If you don't give it to them, that could cause you a really big headache. (this may not be your case) If he is going thru all of this, he could go to court and give up his paternal rights.
